I found that the switch does not require a neutral wire, but if you have one, the fourth wire is for it. I was initially unsure of what to do with it, but eventually figured it out.
If the ground is not connected, the timer switch will not work and it will be always on without any way to control it.

This timer works great with two timing options, a 60-minute timer and a 4-hour timer.
The timer has been installed for 2 years and the only issue is that the 60 minute just recently began blinking and changing to 30 immediately after setting the time.

I suggest grounding the switch by using a multimeter to confirm the electrical box is grounded and then attaching the ground wire to it from the switch lead.
The timer's faceplate is slightly too big to fit in a standard rocker switch faceplate as advertised, and requires grinding down with a dremel.
