Based on my experience with the product, I suggest buying an 18x18 pillow insert if the pillow case size is 16x16.
I purchased 16x16 throw pillow inserts for specific covers, but they turned out to be 15 inches at the most, which is not accurate.
I purchased the inserts with the intention of using them for a euro sham size, but I learned that a euro sham size is 26x26, not 20x20, and the inserts were too small.
